PLEASE READ AND CHECK – I acknowledge that the sport of ice hockey is dangerous, and knowing this, I, and the above named participant, willingly participate in all activities required by MIDWEST GOALIE SCHOOL LLC. I release all employees of MIDWEST GOALIE SCHOOL LLC, and all host rink employees, from responsibility for any injuries that may be incurred against the above named student, on or off the ice in any circumstances related to the above’s attendance at MIDWEST GOALIE SCHOOL. Further, I assume responsibility for the security of all of my possessions including but not limited to hockey equipment, money, or any other valuables. Camp attendees understand that in any activity involving a publicly accessible place and involving other people, there is a risk of exposure to bacteria, viruses, or other communicable diseases (including, without limitation, risks associated with COVID-19), toxins, parasites, or other adverse health factors. Attendees assume the risk of exposure when they attend a camp.
